As an employer, am I responsible for reporting every injury as a workers' compensation claim?
Each injury that results in an employee missing time from work and/or
that requires medical attention should be reported to us on the
"First Report of Injury or Illness" form.
In order to help manage your costs, you should make a report within 24 hours of the
incident. Find out how prompt reporting can save you money.
What kind of records should I keep? Your personnel files should document:
- When the employee first complained of a condition;
- When, where and how the injury occurred;
- What the employee was doing and why, and
- Which part of the body was affected.
NOTE: Many states require early investigation, documentation and reporting of
each injury. Even if you are unsure whether or not an employee's, injury or illness is
work-related, document the details for future reference.

Where should I send my employee for medical care?
State laws vary about choice of physicians for treatment of workers' compensation

What are SICs?
Standard Industry Codes, or SICs, are universal codes that help identify industries by
type. SICs are sometimes required to complete a "First Report of Injury or Illness" form.
